What is an LED lamp?

LED is an abbreviation from the English language and means Light-Emitting Diode. The Led lamp is a device used in Low-Level Laser Therapy (LLLT) delivered by high-intensity light emitting diodes (LEDs). The use of LEDs in photobiomodulation is quite well established, and their effectiveness has been confirmed in many studies.

How does an LED lamp work?

The LED lamp emits specific wavelengths in the visible spectrum and infrared light, which, penetrating the tissue, cause therapeutic benefits. The Celluma lamp emits waves in the range of 465 – 880 nm.

Mechanism of LED

  1. When a light source (LED lamp) hits the tissue, it emits photons
  2. The photons are then absorbed into the mitochondria and cell membranes. Inside the mitochondria is a matrix with enzymes catalyzing Krebs cycle reactions.
  3. Activation of mitochondria increases the synthesis of ATP (adenosine triphosphate, chemical energy carrier)
  4. Increased ATP production causes metabolic events resulting in biochemical and cellular changes.

This entire process is called photobiomodulation. Photobiomodulation affects both fibroblasts, leading to increased synthesis of extracellular matrix components as well as keratinocytes, mast cells, neutrophils, and macrophages.

What biochemical and cellular changes does ledotherapy cause?

Biochemical and cellular changes depend on the length of the emitted light wave.

Blue light 465 nm

Penetrates the epidermis and has a strong antibacterial effect. During irradiation, singlet oxygen is formed. Singlet oxygen damages the cell membranes of microorganisms, e.g., Cutibacterium acnes; hence blue light is most often used in the case of acne vulgaris and rosacea (active phase). In addition, blue – light reduces inflammation and soothes itching, which is highly beneficial in reducing symptoms of psoriasis and atopic dermatitis.

Interesting facts: Both blue and red light (used alone or in combination) are used in treating acne. However, the combination of blue and red light gives even better results, and this type of light is emitted by the Celluma lamp in treatments for acne or psoriasis.

Red light 640 nm

Penetrates the epidermis and dermis. The light absorption in the dermis leads to the proliferation of fibroblasts, the synthesis of collagen, growth factors, and extracellular matrix production. Red light effectively improves the dermal-epidermal junction and modulates expression proteins related to tissue repair. By affecting keratinocytes, it positively affects the structure and color of the skin. Red light absorbers are also mast cells (crucial in inflammatory reactions) and macrophages (responsible for the activation of specific immune responses).

As a result, red light:

  • improves skin elasticity
  • reduces wrinkles
  • relieves inflammation,
  • soothes redness
  • reduces swelling
  • accelerates wound healing
  • has a positive effect on the scar reconstruction process
  • accelerates hair regrowth in male and female alopecia

Near infrared IR 880 nm

It penetrates the deepest layers of the skin, and its essence is to generate heat. Infrared radiation stimulates angiogenesis (forming new capillaries) and reduces inflammation and pain in joints and muscles. Together with red light, it causes the migration of cells: mast cells (crucial in inflammatory reactions), neutrophils (they are the first line of defense in inflammatory processes), and macrophages (responsible for the activation of specific immune responses), thus accelerating wound healing.

Red light + near infrared IR have biological effects on tissues:

  • rising carbon monoxide
  • dilating blood vessels, improving blood circulation
  • reducing swelling and inflammation
  • angiogenesis (forming new capillaries)
  • neurogenesis (forming new nerve cells)
  • increasing cell division (mitosis)
  • inhibiting cell death (apoptosis)

Indications for the Celluma lamp therapy (depending on the selected light length):

  • acne
  • rosacea
  • excessive hair loss, androgenic and telogen alopecia
  • psoriasis
  • seborrheic dermatitis
  • reduction of wrinkles, improvement of skin firmness and density
  • scars
  • wounds
  • burns
  • joint pain, muscle pain, e.g., shoulder pain, neck pain, arthritis, post-herpetic neuralgia, carpal tunnel syndrome (CTS), bursitis, tendinitis, or ankle sprain

Why is Celluma so effective?

For a light therapy device to be effective, a certain amount of light energy must reach the target tissue to achieve clinical benefit. So what parameters will be the key?:

  1. Wavelength – must correspond to the absorbance of the desired photo-accepting molecule and determines the depth of penetration of the light delivered. Celluma emits wavelengths based on clinical research: blue, red, and near-infrared.
  2. The energy density should be high enough to produce the desired effect but low enough not to cause toxic or adverse effects. The acceptable energy range should be in the range of 2-10 J /cm2. Celluma has been designed to meet these requirements. Additionally, Celluma emits pulsating light. It has been shown in clinical trials that pulsed light energy delivery increases penetration depth and the cell’s ability to absorb more energy. In some cases, pulsed light offers additional advantages over continuous frequency.
  3. Pulse frequency – for the Celluma lamp, the pulse frequency is respectively: for blue light 80 Hz, red – 680 Hz, and near-infrared 800 Hz for 30 minutes per treatment.
  4. Duration of treatment – a single session with the Celluma lamp lasts 30 minutes and should be repeated twice a week for several weeks (depending on the indication) for the best results. A more extended treatment gives the body more time to respond to a therapeutic effect. Clinical trials have shown that the most effective treatments are treatments repeated twice a week for several weeks.
  5. Tissue distance from light source – The closer the light therapy device is to the tissue, the more energy is absorbed by the cells. Celluma is characterized by a large, flexible treatment area that can be well-fitted around any body part. The diodes are placed on an elastic panel that can be freely adapted to the tissue at a selected distance.
  6. Emission of several wavelengths at the same time – Celuma simultaneously emits three wavelengths in the correct proportion on each therapeutic program. It is a more efficient and effective method of increasing cell function. Different cells at different tissue depths absorb the needed photonic energy to produce beneficial results.

NASA technology?

The Celluma lamp was created based on NASA technology. In the interest of astronauts who are exposed to injury or disease during prolonged space flight, NASA has conducted a series of studies on the effects of microgravity and hypergravity on the growth of human and plant cells. Gravity has been shown to be essential for proper cell growth. In connection with the search for solutions, NASA has developed light-emitting diode (LED) technology that stimulates the basic energy process in the mitochondria of each cell. It has been documented that DNA synthesis in fibroblasts and muscle cells increases fourfold under the influence of LED light.

Based on NASA research, the Celluma lamp was created, combining LED light energy and patented photomodulation technology.

What are the contraindications to the Celluma lamp therapy?

  • allergy to light, epilepsy,
  • porphyria
  • pregnancy,
  • tumors,
  • taking photodegradable drugs

How often and how many treatments should be performed to obtain satisfactory treatments?

The number of repetitions and frequency are selected individually depending on the case and the patient’s capabilities. Due to the safety of the Celluma lamp, the therapy can be performed until the patient achieves satisfactory results.

Can Celluma be performed as an independent treatment? Is it just a supplement to a complex treatment?

Due to its safety and effectiveness, Celluma can be used as an independent therapy. The effects of a series of treatments are spectacular without implementing a complex treatment. There are treatment protocols in which we can combine the operation of the LED lamp with additional procedures. It is then possible to shorten the therapy time and obtain even better results. It all depends on the case and the possibilities of the patient.
